卡维地洛对羟基代谢物性质、用途与生产工艺
卡维地洛是第3代β受体阻滞剂,由罗氏公司研发并于1991年上市,1995年FDA批准该药用于治疗慢性充血性心力衰竭,1996年开始在世界各国广泛使用。它除了具有阻滞肾上腺素β1和β2受体的作用外,还有阻滞肾上腺素α1受体和阻滞钙、钠、钾通道,抑制平滑肌细胞增殖,抗炎,抗氧化,抑制前列腺素F1a的缩血管作用。其中针对心血管作用的研究最为深入,此外对于肾脏、肝脏的作用及抗氧化的作用等也受到关注。临床研究证明,卡维地洛能显著降低心衰的发病率和死亡率,同时还能防止细胞凋亡的发生,是传统β受体阻滞剂所不能比拟的。因此,卡维地洛最初用于原发性的轻、中度高血压和心绞痛的治疗,也是目前我国批准用于临床治疗的唯一适应症。其降压效果良好,副作用较小,具备了安全、温和降压的特点,是高血压伴有心绞痛或心肌梗塞患者的首选药物。
