World Health Organization (WHO)Berberine, an alkaloid contained in many plants including Berberis species, remains available in many tropical countries. Both traditional herbal remedies and tablet formulations containing this substance have been used in the treatment of gastrointestinal disease, and injectable preparations have been claimed to be of value in the treatment of cutaneous leishmaniasis. The action taken in Singapore relates to reports of jaundice, haemolytic anaemia and kernicterus with brain damage in infants with G6PD deficiency who were exposed either in utero or post-natally. Preparations for topical application are also available in some countries. These have not been associated with reports of systemic toxicity.

Enzyme inhibitorThis yellow isoquinoline-based alkaloid (FWfree-base = 336.37 g/mol; FWchloride = 371.82 g/mol; CAS 2086-83-1) is an acetylcholinesterase inhibitor that also induces apoptosis in promyelocytic leukemia HL-60 cells, modulates the expression and function of P-glycoprotein 170 (multi-drug resistance transporter) in hepatoma cells, and inhibits platelet aggregation induced by ADP, arachidonate, and collagen in rats. It is a fluorescent stain for heparin in mast cells. Berberine inhibits β-catenin transcriptional activity and attenuates anchorage-independent growth, reducing cellular levels of active β-catenin and increasing the expression of E-cadherin. A screen of synthetic 13-arylalkyl berberine derivatives identified compounds exhibiting activities superior to those of the naturally occurring parent substance with more than 100-fold lower EC50 values for Wnt-repression. Berberine is also a partial agonist of a2 adrenoceptors. With rat cerebral cortex synaptosomes, berberine inhibits the release of glutamate evoked by the K+ channel blocker 4-aminopyridine, an effect that can be prevented by the chelating extracellular Ca2+ ions or by the vesicular transporter inhibitor bafilomycin A1. Berberine inhibition is insensitive to the glutamate transporter inhibitor DL-threo-b-benzyl-oxyaspartate. The inhibitory effect of berberine on glutamate release is associated with a reduction in the depolarization- induced increase in cytosolic free Ca2+. Berberine is a component of the Chinese herb medicine Huanglian (Coptis chinensis). Note: Tetrahydroberberine is canadine (See Canadine).
