曲帕拉醇性质、用途与生产工艺

简介

曲帕拉醇是第一种合成的降胆固醇药物。它于 1959 年获得专利，并于 1960 年在美国推出。 Triparanol 的开发代号 MER/29 广为人知，以至于它成为了该药物的注册商品名。 由于恶心和呕吐、不可逆白内障导致的视力丧失、脱发、皮肤病（如干燥、瘙痒、脱皮和“鱼鳞”质地）和加速动脉粥样硬化等严重不良反应，于 1962 年被撤回。

药理作用

该药物通过抑制 24-脱氢胆固醇还原酶起作用，该酶催化胆固醇生物合成的最后一步，即去甾醇转化为胆固醇。虽然能有效降低胆固醇水平，但这会导致去甾醇的组织积累，而这反过来又是造成三聚四醇的副作用的原因。与他汀类药物不同，三丙醇不会抑制 HMG-CoA 还原酶，胆固醇生物合成中的限速酶，与三丙醇相比，他汀类药物可以降低胆固醇水平，而不会导致中间体如去甾醇的积累。
