双氯西林性质、用途与生产工艺
双氯西林为对产酶金葡菌作用最强的一种半合成青霉素,比青霉素强100倍。对不产酶金葡菌、表葡菌以及A族链球菌、肺炎球菌、炭疽杆菌等革兰阳性菌均具抗菌活性,但都不及青霉素;对粪肠球菌作用弱或无效。
口服吸收率好,生物利用度60%,受食物影响,空腹口服0.59,Tmax 1h,Cmax为24mg/L,血药峰值几近苯唑、氯唑西林的2~3倍;代谢0.8h,肾功能不全者延长;PPBR为97%;在体内分布广,类似氯唑西林,脑膜炎时可少量进入脑脊液中。小部分药物在肝脏代谢,服药6h约60%随尿排出,少量经肝随胆汁排出。不为血、腹透清除。
耐青霉素而对本品敏感的葡萄球菌引起的败血症、心内膜炎、呼吸道及皮肤软组织细菌感染等。也可用于化脓性链球菌或肺炎球菌与耐青霉素的葡萄球菌所致的混合感染。但本品不适用于MRS感染。
